The practice that started it all.
The Kidney Holistic Institute is the practice Dr. Irfan built first — the one she designed before there was a name, a website, or a method called HEAL.
For most of modern medicine, kidney disease has been treated as a one-way trajectory. Numbers worsen. Stages advance. Patients are told to manage symptoms, monitor trends, and prepare for what comes next. Dr. Irfan trained inside that system — at Albert Einstein College of Medicine, at Cleveland Clinic Lerner College — and watched it leave the most important question unanswered: why.
The Kidney Holistic Institute is the answer she built. The premise is simple and unfashionable in conventional nephrology: your kidneys are signaling, not failing. The lab values that bring patients here are the body’s voice, three steps after the upstream conversation began. Find that conversation. Address it. The labs follow.
What we treat.
Five primary conditions account for most of the work. We see them at every stage — including patients told there was nothing left but dialysis — and the methodology adapts to the moment in the disease, not the diagnosis on the chart.
- 01Chronic kidney disease (CKD) — all five stages, including marker improvement in early stagesMost common
- 02Hypertension — renovascular and uncontrolled, addressing root driversFoundational
- 03Diabetic nephropathy — metabolic and microvascular, parallel to glycemic workCommon
- 04Lupus nephritis — alongside conventional immunosuppressionComplex
- 05Polycystic kidney disease (PKD) — slowing progression, supporting declineGenetic
Upstream drivers we treat
The conditions above are downstream labels. The work happens on the systems below them.
The HEAL Protocol.
A four-pillar framework Dr. Irfan developed across 14 years of clinical nephrology and a thousand patient cases. It is the method this practice runs on — and the one she teaches now to other practitioners.
Each pillar is a system in its own right. Together they form a sequence: restore the body’s signals first, remove what is blocking repair, rebuild the foundation the kidney depends on, and consolidate gains in daily life. Most patients begin in the first two pillars and revisit all four cyclically.
The full description of each pillar — the markers we measure, the interventions we use, the order in which we sequence them — lives on the methodology page.

How an engagement works.
The kidney work runs in a defined arc — not because every patient needs the same thing, but because the sequence has been refined over a thousand cases. Most patients start where they need to start; the rest of the protocol unfolds from there.

Where to continue.
This page is a doorway. The kidney work is documented in three places that go deeper than a single page can — the dedicated practice site, the book, and the weekly podcast. Each is meant for a different moment.

Frequently asked.
Can chronic kidney disease be reversed?
I am careful with that word. My premise is that kidneys are signaling, not simply failing — and that in earlier stages we can often improve markers and slow or stabilize progression by addressing the upstream drivers. For advanced disease the goal shifts to protecting function and quality of life. This is added alongside conventional nephrology, never instead of it.
What kidney conditions do you treat?
Five account for most of the work: chronic kidney disease across all five stages, hypertension (including uncontrolled and renovascular), diabetic nephropathy, lupus nephritis alongside conventional immunosuppression, and polycystic kidney disease. The method adapts to where someone is in the disease, not just the label on the chart.
What is the HEAL Protocol?
HEAL is the framework I built for kidney care that works on the systems beneath the diagnosis — chronic inflammation, heavy metals and toxins, hormonal imbalance, gut dysfunction, metabolic and mitochondrial health, and nervous-system stress. The lab values that bring patients in are treated as the body’s voice, and we trace them back to what is driving them.
Do you replace my nephrologist or dialysis care?
No. This is functional, root-cause work that runs alongside conventional nephrology — including for patients on or approaching dialysis. I work with your existing care, not against it, and I am transparent about what the approach can and cannot do.
